The FM transmitter is a "nice to have" if you're going round to someone else's house and want to play songs to your friends. However, as you no doubt know by now, in a car it is pretty hopeless since you lose your 'station' all the time and there is a huge amount of interference.
I bought the Apple accessory that plugs in between headphones and iPod which allows you to skip tracks, change volume and play/pause without having to use the iPod itself. This could prove useful for you in the gym where you don't want to have to keep getting the iPod out to stop/start it etc. with sweaty palms.
Avoid the "iSkins" because although they sound nice in theory, in practice they get covered in fluff and dust, thereby making your prized possession look horrible and nasty to touch. If you want to protect it, get a leather case - or just no case if, like me, you're not bothered about scratches and prefer the feel of a naked iPod.
